Our study was born following the complex work of the operators in the Operating Room and in the Intensive Care Unit, where there is a use a huge amount of drugs administered intravenously in bolus and / or continuous infusion.
The administration of drug is a complex process of actions with several phases, that increase the risk of error.
Furthermore, due to the dosage, instability of the critical patient, all intravenous drugs used in intensive care, necessary for critical patient care, can be considered high risk and for this reason, potentially dangerous.
Improper use of drugs can lead to adverse events with serious consequences for patients.
Adverse events due to errors in "drug therapy" are the most frequent cause of damage in hospitalized patients and can occur throughout the drug management process.
These preventable and avoidable events should be differentiated from Adverse Drug Reaction (ADR), related to the drug itself and that are detected and evaluated through national, regional or company pharmacovigilance activities.
Therefore, in the context of hospital assistance, it becomes a priority to prevent the occurrence of the sentinel event due to incorrect use of drugs with particular reference to events related to the use of drugs considered to be "high risk".
Furthermore, the exchange of information with colleagues and other professionals during the course of therapy is sometimes reduced due to urgency.
